Alcohol Approval Request

Stetson University requires the sponsoring organization to read the attached alcohol policy and guidelines, and to complete the alcohol approval form at least seven days prior to the event. The application will be reviewed by staff in the University Events Office with a signed copy of the decision (approved or denied) to the Event Sponsor, University Risk Manager, Vice President of Business and CFO, and Sodexo Dining Services.

Agreement

The sponsoring organization/person agrees to comply with Florida State Laws, local ordinances, and Stetson University's policies, including the University's Alcohol Policy. It is understood that the sponsoring organization is responsible and accountable for the actions of the guests at this event. The sponsoring organization assumes all responsibility for damage to University property.

It is understood and agreed that Stetson University bears no responsibility for the purchase, distribution, damages, injuries or effects of alcoholic beverages served or consumed at events held on the premises of or areas adjacent to the University.

Distribution, sales and serving of alcohol on campus is only permitted through a third party vendor with a valid liquor license, trained servers and documented liability insurance to include a liquor liability policy. Third party vendor must submit a certificate of insurance reflecting current liability policies and must name Stetson University as an additional insured in regards to liability and their services. Third party server(s) must be present at all times; alcohol cannot be left unattended. The third party vendor is responsible for the sales, serving and supplying the wine, beer, and liquor served at the event. The third party alcohol vendor is also responsible for checking legal age of patrons/attendees as well as sobriety judgment in regards to serving alcohol to attendees.

As an authorized representative of the sponsoring organization, I agree to the above and accept responsibility on behalf of the organization. I agree to remain in attendance throughout the entire event. I have received, read, and will adhere to the University Alcohol Policy and Guidelines.
